St. Anthony Health & Rehabilitation provides long-term care when the individual’s needs exceed a family’s ability to provide care at home. Here you will find experienced nursing team members who are dedicated to provide high-quality care. Professional team members, including registered and licensed practical nurses and certified nursing assistants, are on duty 24 hours a day. Residents are also seen regularly by their personal physician.
Our residents’ families are an extremely important aspect of our care model. We encourage and support family members to visit and participate in the daily lives of our residents. Compassionate care is provided and individual dignity is promoted. We strive for an all-around homelike environment.
Our rehabilitation and wellness services are designed to help you return home as soon as possible. Highly experienced nurses and therapists provide care that’s both physically and emotionally healing. Each resident who enters our rehabilitation and wellness program receives a customized treatment plan based on their specific medical condition and wellness goals. Team members carefully measure progress at regular intervals and make adjustments to promote improvement in physical strength, health, and endurance.
Physical therapy helps with the treatment of pain, disease, or injury. Our physical therapist team seeks to maximize quality of life.
Our occupational therapists work with people of all ages who need specialized assistance to lead independent, productive, and satisfying lives. Occupational therapists use the “occupations” of self-care, work, and play/leisure activities to increase independence in activities of daily living such as dressing, self-feeding, bathing, and grooming.
Speech and language therapy can enhance a person’s ability to speak effectively, as well as regain lost abilities to communicate, comfortably eat, drink and swallow.
Outpatient rehabilitation is a form of therapy where patients attend rehabilitation sessions for their series of treatments. When patients are admitted into outpatient programs, it shows that the patients are doing well enough to live at home. Our rehabilitation teams work to develop individualized treatment plans to help patients reach their goals.
St. Anthony Health & Rehabilitation’s memory care community offers Montessori Inspired Lifestyle®, a positive approach to providing care for adults living with dementia or Alzheimer’s disease. This approach of care is based on the methods of Maria Montessori, as adapted for adults by Dr. Cameron Camp, founder of the Center for Applied Research in Dementia. Dr. Camp is the innovator and founder of the Montessori Inspired Lifestyle Approach to Dementia Care. This programming focuses on individuals’ strengths and promotes opportunities for success each day. Our staff’s expertise in working with memory care residents helps make the most of every moment.

When end-of-life care is needed, St. Anthony Health & Rehabilitation partners with hospice agencies to provide comfort-focused care. Hospice care is offered when an individual’s care needs turn from cure to comfort. This care can be provided in each individuals’ apartment or room. This patient-focused care offers numerous services to help manage pain and symptoms, supportive services to align your care goals as well as support to family and loved ones.
